Usually, the business which makes the association with any vendor demand for the reference form from any of the clients of the vendor. The reference form provided by the client or the customers is then checked by the managers of the company through various ways and then the performance of the vendor is evaluated.   1. Name of the client
  2. The name of the person who is submitting the reference form
  3. Name of the company
  4. Customer’s phone number and email address
  5. The description of the services that the vendor has provided to the client
  6. The satisfaction level of the customer is also explained in the form
  7. A separate field is also included in the vendor reference form which allows the customers to give some extra comments about the vendor
  8. The delivery of the product or the services details
  9. The starting and ending date of the contract with the vendor

All these fields in the form are sufficient to check the performance of the vendor.
